Fall 2017 Registration

On-time registration for the Fall 2017 season will begin May 8, 2017 and run until August 12, 2017. After that date, we will continue with late registration until the season begins or all spots are filled.

Please note that we will be using the uniform from the Fall 2016/Spring 2017 seasons. If your child played in the fall 2016 and/or the spring 2017 season and remains on the same team, he/she will be able to use their uniform and pay the returning player fee. If your child is new to the league, wants to change teams, or just need a new uniform, you will need a new uniform and must pay the new player fee.

Registration Fees:
   New Player:
      *Single Child ....................$150            
      *Multiple Children ...............$145 ea.  
      *Head Coach's Child ..............$ 70            
      *Assistant Coach’s Child .........$120        
         *Registration Fee includes a full uniform for children (jersey, shorts
          and socks). 

   Returning Player:
       Single Child ....................$125             
       Multiple Children ...............$120 ea.  
       Head Coach's Child ..............$ 50             
       Assistant Coach’s Child .........$100 

If you mail your registration after the August 12 dead-line, you must also 
   include the $25 late fee for each child. Faxed and emailed forms cannot be 
   processed until the registration fee is received.

1. Print out the pdf form, then mail the form with the registration and a 
   check or money order to:

2. Print and fill out the pdf form, and bring it to our office at:

   where you can drop off the registration form(s) and pay by credit/debit 
   card, cash, check, or money order.

3. Print out the pdf form, fill it out, scan it, then attach the registration 
   form to an email and send it to, register@seminoleyouthsoccer.com. Then 
   call 407-324-5354 with a credit/debit card payment.

4. Print out the pdf form, fill it out, and fax it to 407-804-0051. Then call 
   407-324-5354 with a credit/debit card payment.
